Hospice Social Worker Salary : $65-$78k/yr (BOE) Schedule : M-F| 8:00AM-5:00PM (on-call rotation) Location : Hilliard, Ohio and surrounding area Requirements of the Hospice LSW role: Licensed Social Worker in state of practice (LSW) Master’s Degree in social work preferred (MSW) One year of social work experience in a healthcare setting is preferred Current Driver’s License, vehicle insurance, and access to dependable transportation. About the Hospice LSW ro le: Provide medical social services to the patient and/or family according to the Plan of Care. Conduct a psychosocial assessment as part of the comprehensive assessment within 5 days of admission to the hospice program, including emotional, social, financial, and environmental resources, and identify appropriate psychosocial problems, interventions, and goals for the patient’s care. Participate in IDG meetings and activities and the development of the patient plan of care. Observe, record, and report changes in the patient’s emotional and social factors that are being affected by the patient’s illness and his/her need for care and response to treatment. Maintain and submit written clinical records as deemed by Care 360 Hospice, including the initial evaluation, the care plan, and daily notes.
