Auburn Crest Home Health & Hospice On-Call Registered Nurse (RN) – Home Health & Hospice Auburn Crest Home Health & Hospice is seeking a compassionate and dependable On-Call Registered Nurse (RN) to join our team. This role includes after-hours telephone triage from 5:00 PM to 8:30 AM, providing in-home visits as needed, documenting and communicating patient updates to case managers, and supporting patients and families during critical moments of care. We are looking for a nurse who is confident working independently while also collaborating closely with our interdisciplinary team to provide exceptional home health and hospice care.
