Position Highlights Part-time schedule – 4 days per week Daytime hours with no evening clinic responsibilities Manage and round on patients in a Skilled Nursing Facility Collaborative physician-led team Established patient panel Administrative support and streamlined documentation Competitive compensation Responsibilities Perform daily inpatient medical rounds in a Skilled Nursing Facility. Evaluate, diagnose, and manage acute and chronic medical conditions. Develop and implement individualized treatment plans. Coordinate care with physicians, nursing staff, therapists, specialists, and family members. Complete timely and accurate documentation within the EMR. Participate in quality improvement initiatives and ensure compliance with facility and regulatory standards. Provide compassionate, patient-focused care that promotes positive clinical outcomes and reduced hospital readmissions. Qualifications Current Nurse Practitioner license in the state of practice. Excellent communication and clinical decision-making skills. Ability to work independently while collaborating with an interdisciplinary care team.
