POSITION SUMMARY **Successful candidate should have experience in OB/GYN and Pediatrics.** This Registered Nurse functions to assist all patients and their families through the healthcare system by acting as a health educator and patient advocate. They will also work with patients to develop preventive and self-management goals to improve their health. Additionally, this nurse position may provide triage services to patients calling for urgent care needs to provide medical advice and identify if outpatient services are appropriate or if they should be escalated to a higher level of care. This position is essential to Adelante’s team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.
