Spanish-Speaking Social Worker – Home Care Intake Location: Hempstead, NY Salary: $90,000–$110,000 per year A reputable healthcare provider in Hempstead is seeking a Spanish-speaking Social Worker to join their team to support home care patient intake . This full-time, in-office position involves evaluating the needs of incoming home care patients and helping coordinate their care with compassion and professionalism. Key Responsibilities: Conduct intake assessments for new home care patients to determine their individual needs and eligibility for services Develop and maintain individualized care plans to ensure patients receive appropriate and timely support Collaborate with physicians, nurses, and other providers to deliver coordinated, high-quality care Communicate directly with patients and families to provide education, counseling, and emotional support Maintain accurate documentation in compliance with regulatory and facility standards Follow up with patients regularly to ensure continuity of care and adjust services as needed Qualifications: Must be certified/licensed as one of the following: LCSW, or LMSW Fluency in Spanish is required to effectively serve the patient population Prior experience in home care, social work, or patient intake is strongly preferred Excellent communication, interpersonal, and organizational skills Empathetic, patient-centered, and able to work independently as well as collaboratively Additional Information: Please note that the practice may relocate in the future to either Woodmere or Cedarhurst, NY.
