We currently provide home health care to patients in Santa Clara county, San Mateo County and San Benito county, and are expanding into Alameda, Contra Costa & Solano counties.  Ascension Healthcare has an immediate need for a Home Health Physical Therapist in Santa Clara county, Sant Mateo County, San Benito County, Alameda County, Contra Costa County and Solano County.
