Duties and Responsibilities Assess patient’s health problems and needs, develop and implement nursing care plans, and maintain medical records. Administer nursing care to ill, injured, convalescent, or disabled patients. Advise patients on health maintenance and disease prevention or provide case management. Liaise with physicians and other team members to plan and evaluate patient care plans. Modify patient treatment plans as indicated by patients' responses and conditions. Guide and supervise health care personnel like LPNs and home health aides. Manage a caseload of patients, ensuring all patients receive the required care as per the established guidelines. Maintain accurate, detailed reports and records of patient conditions and treatments. Monitor, record, and report symptoms and changes in patients' conditions in a timely manner.
